Robert Saddawi-Konefka, MD, PhD is an Assistant Professor of Otolaryngology–Head and Neck Surgery at University of California San Diego and a surgeon-scientist whose work focuses on how the tumor–lymphatic–immune axis shapes response to cancer immunotherapy. His research addresses a central limitation in the field: despite the success of checkpoint blockade, response rates in head and neck cancer remain limited, and combinations with standard therapies have not consistently improved outcomes. His work defines a mechanistic basis for this gap. While conventional treatment prioritizes aggressive locoregional control, including removal or irradiation of lymphatic basins, his studies show that tumor-draining lymph nodes function as essential immunologic hubs where antitumor immunity is initiated and sustained. Disruption of this axis compromises immune priming, effector expansion, and the development of durable responses. Using inducible lineage tracing, spatial profiling, and single-cell approaches, his work has mapped the spatiotemporal dynamics of immune cell trafficking from tumor to lymph node. These studies defined the tumor–sentinel lymph node immune migratome, a coordinated program of dendritic cell migration, antigen presentation, and T cell activation required for effective immunotherapy. Early disruption of this program through lymphatic ablation abolishes checkpoint inhibitor efficacy and prevents formation of immune memory. These findings have informed ongoing clinical trials testing lymphatic-preserving treatment strategies, with early signals of enhanced pathologic response. In parallel, his work advances biomarker development through peripheral immune monitoring approaches, including the LiBIO platform, which captures dynamic immune responses in circulation to predict therapeutic outcomes. Dr. Saddawi-Konefka is a member of the Moores Cancer Center and the Gleiberman Head and Neck Cancer Center at UCSD. He completed his MD/PhD and residency at UCSD and fellowship training in head and neck surgical oncology at MD Anderson Cancer Center.
